Healthcare providers delivering services through The Hormone Experts are licensed medical professionals responsible for the medical care they provide.

In many cases, telehealth services require synchronous audio-visual communication between the patient and provider, unless otherwise permitted by applicable law.
Provider Relationship
A provider–patient relationship is established only after:
• Completion of required intake forms
• Review of medical information by a licensed provider
• Completion of a telehealth consultation when required by law
Treatment decisions, prescriptions, laboratory testing, and therapies are issued only when deemed medically appropriate by the provider.
Not all patients will qualify for all treatments.

Information available on the Platform is provided for educational and informational purposes only.
Content on the Platform is not intended to replace professional medical advice, diagnosis, or treatment.
Patients should always consult with a qualified healthcare provider regarding medical concerns or treatment decisions.

Eligibility
You must be at least 18 years old to access or use the Platform.
By using the Platform, you represent and warrant that you meet this age requirement and are legally capable of entering into binding agreements.
Patient Location and Licensing

By using the Platform, you acknowledge that:
• You must be physically located in a state where the provider is licensed to practice at the time of the telehealth consultation
• Providers may request confirmation of your location prior to or during a consultation
• Services may not be available in all jurisdictions
The Company reserves the right to limit or deny services based on provider licensing and applicable state regulations.
User Responsibilities
Users of the Platform agree to:

• Maintain the confidentiality of their account credentials

You are responsible for all activity occurring under your account.
Prescription Policy
Licensed healthcare providers determine whether medications are appropriate based on a patient’s medical evaluation.
If a prescription is issued:
• Medications may be fulfilled by licensed pharmacies
• Prescriptions are issued only when medically appropriate
• Certain medications may require laboratory testing or follow-up consultations
The Company does not guarantee that a prescription will be issued.
Membership and Payments
The Hormone Experts operates as a cash-pay medical practice and does not bill insurance.
Some services may be offered through membership-based subscription programs that provide ongoing access to clinical services.
By purchasing services or memberships, you agree to:
• Pay all applicable fees associated with services received
• Provide accurate billing information
• Authorize payment processing through secure third-party payment processors

You may not copy, reproduce, distribute, or exploit any content without prior written permission.
Platform Availability
